Use form SC9 if you're an employee, are adopting a child from abroad and need to apply for Additional Statutory Paternity Pay (ASPP) and Leave.

There is no longer a requirement for this form as Additional Statutory Paternity Pay and Leave was replaced by Shared Parental Leave and Pay for babies due or children placed for adoption on or after 5 April 2015.
