This notification was originally issued in November 2024. The revised version provides clarification on the regulator’s determination regarding the quality failure and its implications for compliance with the code.

The Forensic Science Regulator notification has been issued to make the forensic science community, and criminal justice system aware of a procedural error related to the use of certified reference materials in the undertaking of FSA- DTN 102 and the findings of an independent review commissioned by the FSR to assess the significance of the error.
