** Action 1** - Performance Report Q3 2025–26 (FSA BC 26/03/04) - Operational Delivery Junior Johnson to provide further detail on the specific issues identified at the repeatedly non-compliant raw drinking milk farm, including any learning relevant to other producers. 8 June 2026 Junior Johnson – Complete: There were challenges in finding the root cause of the issue and these were causing the repeated failures.  In the end the root cause was found by the FBO following a detailed investigation and we have shared the learning from this case with industry through our stakeholder engagement pathway.  The sales at this farm remained ceased throughout the entire time period that the investigation was being completed.
Action 2 - Performance Report Q3 2025–26 (FSA BC 26/03/04) - Operational Delivery Operations and NFCU teams to develop and provide the Business Committee with a more systematic set of outcome-focused indicators for NFCU activity, including economic impact, key enforcement outcomes, and progress of cases affected by court delays. 8 June 2026 Junior Johnson – Ongoing: There has been an ongoing project with SERD around how we better demonstrate the impact of the NFCU. Stage 1 concluded at the end of the 25/26 financial year and the FSA’s ACSS group have provided recommendations around the feasibility of more comprehensive impact metrics to be created for the NFCU and have offered an idea of a conceptual model. The NFCU Senior Managers are considering proposals that will be presented to Operations Director for consideration.

Outside of this, work is ongoing around better communicating key NFCU enforcement outcomes and those cases affected by court delays. These will be part of performance reporting in the new financial year
Action 3 - Performance Report Q3 2025–26 (FSA BC 26/03/04) - Science and Evidence Science Directorate to continue development of outcome-focused science metrics and reflect these more clearly in future updates and in the Annual Science Report. 8 June 2026 Julie Pierce – Ongoing: The Annual Science Performance and Impact Report has now been discussed with the Chief Scientific Adviser and an approach agreed.  Currently producing a first draft that will be finalised for submission to Business Committee in September (and annually at the same time each year).  Once this first annual report has been discussed the metrics which are useful to provide on a quarterly basis will be determined.
Action 4 - 2026/27 Annual Plan and Budget (FSA BC 26/03/05) Executive to adapt Business Committee reporting so that Members receive clearer visibility of delivery against major change programme milestones within the annual plan. 7 September 2026 Ian Gibson – Ongoing: This is in hand and we will see the first set of this sort of reporting from Q1 against the Plan for 26/27.  Targeting Business Committee in Sept.
Action 5 - Local Authority Performance Update (FSA BC 26/03/06) Local Authority teams to explore further analysis linking FTE resource levels to business numbers and population, to support strategic understanding of system pressures. 8 June 2026 Nathan Barnhouse and Nathan Phillipo - Complete: The performance management team are currently testing a comparative modelling approach developed by SERD colleagues to create clusters of local authorities with similar characteristics such business numbers, risk levels, geographic size, and deprivation levels. These resulting clusters, and any given LA’s position within the cluster, could be compared with FTE levels.

Including FTE level as a factor to develop the clusters was considered and rejected as FTE levels would be a causal factor that is within the LAs control, rather then the other factors that are fixed and allow for more meaningful comparison for the purposes of performance management.

Once this process has been refined and tested, it can be presented to the Business committee.
Action 6 - CCP Sandbox Progress Report (FSA BC 26/03/08) Sandbox team to include in future reports a clear breakdown of time during which applications are with the FSA versus with applicants, to support Committee oversight of delivery pace. 8 June 2026 Tom Vincent – Complete: Update will be included in future business committee papers.

Action 3 – Chief Executive’s Report to the Business Committee (FSA BC 25/06/03) Darren Whitby to provide an update for the Board via a weekly circulation on progress of the UKHSA led working group on improving protocols for handling incidents in healthcare settings. Autumn 2025 Darren Whitby – Ongoing Limited progress on this work since the UKHSA-led meeting in April, due to summer priorities and resource gaps, we have requested further updates on the work and clarity on UKHSA prioritisation.  FSA team has recruited to resolve resource gap and will seek progress with UKHSA.  We will update when working group reconvenes
