Guidance for medical professionals dealing with leave for restricted patients

Details

This report should be filled in by the clinician responsible for the care of the restricted patient. You can also use it to report changes in the patient’s circumstances.

You’ll only need to complete the form if asked by the patient’s Mental Health Casework Section.

The Secretary of State must have an up-to-date report on all previous leave before considering any new requests for leave.
