Details
If a medical referee asks for a post-mortem examination, you must complete this form afterwards.
The form enables you to give information about the cause of death.
It also asks you to confirm there’s no need for toxicological analysis or an inquest, so the body can be released for cremation.
This file should be downloaded to complete using a programme such as Adobe Acrobat.
