I understand that where I am not the premises licence holder, I must serve a copy of this form on the premises licence holder within 48 hours after giving notice to the Licensing Authority, and directing the licence holder to return the original premises licence to the licensing authority within 14 days.
I understand that failure to give notice to the licence holder will render this notice invalid.
