I/we understand it is an offence, liable on conviction to a fine up to level 5 on the standard scale, under section 158 of the Licensing Act 2003, to make a false statement in or in connection with this application.
I understand that completing this consent form alone will not constitute a transfer application, and in order for the licence to be transferred a separate application must be made by the prospective holder of the licence.
I am aware that the proposed new holder of the premises licence will still need to complete the separate 'application to transfer a premises licence', and that this form by itself is not sufficient to transfer the licence.
