If you can't submit the documents digitally you can send copies by post instead. We'll give you the address and reference number to quote once you've paid the licence fee.
Evidence of the permanent residential address and National Insurance number of the proposed Licence Holder Evidence of the Licence Holder’s permanent Address must be supplied with the Application. This may be a copy of a driving licence; a recent bank or building society statement; a recent utility bill, etc. Evidence of the Licence Holder’s National Insurance number must also be included. This may be a copy of any official document showing the Licence Holder’s name and National Insurance number.

Provide the date that the property became licensable Provide the ‘relevant date’ at which the property, that is the subject of this application, became licensable or provide the proposed date of occupation if the Licence is being applied for in advance. Note: If you are unsure of the ‘relevant date’ provide a breakdown of occupation of the property since 1st April 2006.
Fire Alarm System installation and commissioning report. If the system was installed more than 12 months ago I will also need to have a current test report.
Fire Alarm test Certificate Please provide your most recent test certificate. If the system was installed less than 12 months ago, we will accept the installation certificate instead.
Fire Risk Assessment
Emergency Lighting installation and commissioning report. If the system was installed more than 12 months ago I will also need to have a current test report.

Valid Landlord’s Gas Safety Certificate Provide a copy of your most recent Gas Safe Certificate
Electrical Installation Condition Report (ECIR) Provide a copy of a valid 'SATISFACTORY' test certificate for the electrical installation. These are normally valid for up to five years.
Valid Portable Electrical Appliance Test certificate (“PAT test”) Any portable appliances which you provide to your tenants need to be tested if they are older than one year old.

Proof of Redress Scheme Membership
Proof of Client Money Protection Scheme Membership
Planning Consent, Building Regulations Approvals or Completion (optional)
Service contract for the Alarm and Fire System. (optional)
