What if I can't submit all of these documents?

You don't have to submit optional documents, but it may improve your chances of a successful application if you do.

HACCP (optional)
Provide a copy of the current HACCP for the production requiring a change of approval

Training Records (optional)
Evidence of training of key staff e.g. Level 3 for supervisors, HACCP training etc

I hereby apply, as food business operator of the establishment detailed in Part 1, for approval to use that establishment for the purposes of handling products of animal origin for which Regulation (EC) No. 853/2004 lays down requirements, as set out in the relevant Parts of this document.
