Use this form to make changes or update your test centre’s details.

You need to apply to make changes to:

  • your trading company or legal entity
  • authorised signatory, site manager or deputy manager
  • your site, third-party access, operating hours, classes of vehicles your site can test, and your site equipment

How to apply

  1. Download and save the application form on your computer or device before you start to fill it in

  2. Open the form on your computer or device, fill it in and save it again

  3. Email the form to contract4atf@dvsa.gov.uk. You need to attach any documents to support your application.
