This order was made in relation to a complaint that Mr Adamson breached the fundamental principle of Professional Competence and Due Care and/or Professional Behaviour and/or Objectivity of the Ethics Code for Insolvency Practitioners because;
a) He failed to issue the debtors with a formal notice of a meeting of creditors on 21 October 2013. b) There was an administrative failure to log and record a proxy form, as a result of which a proxy that would have materially affected the outcome of the meeting of creditors was overlooked.
