Schools can ask parents to fill in this form to determine how many of their pupils attract free school meals and the pupil premium. The information parents include should help you see which children are likely to be eligible.

You should then share the completed forms with your local authority. They can run the necessary checks to confirm which children are eligible.

This form is a model - you are free to adjust it to meet your needs.

Parents who want to register their children for free school meals should apply through their local authority.
