People who are eligible to book a COVID-19 vaccination via the NHS national booking service are being sent letters inviting them to book their appointments. This page provides the booking information provided in those letters in a number of alternative formats. A COVID-19 vaccination information leaflet for eligible adults is available.

Alternative formats

A braille version of the English letter is available to order.

British Sign Language (BSL) videos of the invitation letter and of the consent form are available to view or download.

An audio version and large print version are available to download.

Translated versions of this letter are available in the following languages

Albanian, Arabic, Bengali, Chinese, Farsi, Gujarati, Hindi, Kurdish, Nepali, Panjabi, Polish, Romanian, Somali, Spanish, Tagalog and Urdu.
