These documents are for anyone receiving drug and alcohol treatment in England.
We need your consent to collect information about you and your treatment. We hold your information in the National Drug Treatment Monitoring System (NDTMS) database.
We collect information about you to help improve your:
- treatment and other support
- health and wellbeing
- education and job prospects
- family life
You can use the information and consent form to agree to share your information with us. It tells you about:
- what information we collect
- why we collect your information
- why we might share your information
- what your rights are
Your treatment service should share the information and consent form with you at your first appointment, so that you understand how we use your information, and what your options are.
The privacy notice describes in detail how we collect and use personal information about people in drug and alcohol treatment. This is in line with the UK General Data Protection Regulation. The privacy notice includes details on:
- what we do with your information
- how we protect your information and keep it secure
- how to contact us or make a complaint
