Monitor has taken action, under the new licence for providers, to ensure that the Trust deals with the continuing issues it faces.
19 June 2018: NHS Improvement has lifted some of the regulatory undertakings placed on Southern Health NHS Foundation Trust in 2016 after an independent review showed progress in a number of areas. Following the independent ‘Mazars’ report into a number of serious failings in the way the trust investigated the deaths of vulnerable patients, NHS Improvement took action requiring the trust to implement all recommendations in the report. NHS Improvement has also refreshed the remaining undertakings in place at the trust to ensure they reflect the work/ improvements that remain outstanding.
These undertakings supersede the undertakings relating to governance that were accepted by NHS Improvement on 16 April 2014 and the undertakings accepted by NHS Improvement on 30 June 2016.
5 April 2017: We have issued a compliance certificate in respect of paragraph 2 (Former Oxford Learning Disabilities Trust (“the Ridgeway”) Services) of the Trust’s Enforcement Undertakings accepted by NHS Improvement on 17 April 2014. NHS Improvement is satisfied that paragraph 2 of the undertakings has been completed in its entirety.

We’ve asked the trust to make urgent patient safety improvements and address the issues found by the CQC. The additional licence condition would allow us to make management changes at the trust if progress isn’t made on fixing the concerns raised. The warning notice issued by the CQC identifies issues with how the trust monitors and improves the safety of its services, and how it assesses and manages any risks to its patients.
6 April 2016: Following a CQC warning notice, we intend to put an additional condition on the trust’s licence to provide NHS services to ensure it makes patient safety improvements and which would allow us to make management changes at the trust if it doesn’t make progress on fixing the concerns raised.
24 March 2016: As part of our ongoing regulatory action at Southern Health NHS Foundation Trust, we have appointed Alan Yates as an Improvement Director to help support the trust to improve its services for patients.
16 June 2015: NHS Improvement issued a certificate of compliance in respect of paragraph 1 of the trust’s enforcement undertakings accepted by Monitor on 17 April 2014. Monitor is satisfied that paragraph 1 of the undertakings has been completed in its entirety.
