You can use this form as an application by a public company for re-registration as a private unlimited company if you have:

a prescribed form of assent authenticated by or on behalf of all the members of the company
-
a printed copy of the company’s articles as proposed to be amended
A fee of £20 is payable to Companies House in respect of an application to re-register. Make cheques and postal orders payable to ‘Companies House’.
Forms need to be printed at full size on white A4 sized paper.
