This plan tracked the milestones of the department’s ‘Health and Care Transition Programme’.

The plan was also known as the ‘Transition Roadmap’. It provided transition boards with an overview of the implementation of the reforms, outlined in the Health and Social Care Act 2012, from April 2012 to 31 March 2013, when the programme closed.

The ‘Business Case for the Health and Care Modernisation Transition Programme’ was published on 18 July 2013.

The business case was produced in September 2011, following the government’s listening exercise during the passage of the Health and Social Care Bill.

In May 2012, the department published a ‘Transition Scheme for Publication’. In this it made a commitment to publish the business case once the transition programme closed in April 2013.
