This was a request for the number of medication errors reported to NRLS in 2015-16, the number of those errors committed by nurses and the factors/reasons for committing those errors.

NHS Improvement held details of the number of medication errors reported but withheld this information under section 21 of the FOI Act (information accessible by other means). NHS Improvement withheld the remaining information under section 12 of the FOI Act (cost limit exceeded).
