Fish and shellfish (live): model health certificates
Competent authorities of exporting countries should use the relevant model health certificate as a template to create a version exporters can apply for to export live fish, shellfish or aquaculture products to Great Britain, the Channel Islands or Isle of Man.

Details
You must not directly copy the model health certificates provided on GOV.UK. Competent authorities should create their own official documents for use by exporters. These should include all the information from the model certificates.
Any consignments into Great Britain will be considered non-compliant and will be rejected if they have a model certificate that has been directly downloaded from GOV.UK, shows the ‘model certificate only’ watermark and is dated 1 April 2023 or later.
From 1 January 2021, live fish, shellfish or aquaculture products exported to Great Britain must be accompanied by the relevant health certificate. Exporters in EU and non-EU countries should read the guide on how to complete a health certificate
Certificates GBHC059X, GBHC059E, and GBHC670
Non-EU countries must use certificate GBHC059X from 1 January 2021.
EU countries must use certificate GBHC059E from 1 January 2021.
All countries must use certificate GBHC670 from 30 April 2024.
Model health certificate for the import of aquaculture animals for farming, relaying, put and take fisheries and open ornamental facilities (AQO).
Certificates GBHC060X, GBHC060E, and GBHC671
Non-EU countries must use certificate GBHC060X from 1 January 2021.
EU countries must use certificate GBHC060E from 1 January 2021.
All countries must use certificate GBHC671 from 30 April 2024.
Model health certificate for the import of ornamental aquatic animals intended for closed ornamental facilities (AQC).
Certificates GBHC061X, GBHC061E, and GBHC672
Non-EU countries must use certificate GBHC061X from 1 January 2021.
EU countries must use certificate GBHC061E from 1 January 2021.
All countries must use certificate GBHC672 from 30 April 2024.
Model animal health certificate for transit/storage of live aquaculture animals, fish eggs and uneviscerated fish intended for human consumption (AQ-T/S).

GBHC158 must be used from 1 January 2021.
GBHC674 must be used from 30 April 2024.
