Additional Statutory Paternity Pay and Leave: death of the mother or adopter (SC10)

Use form SC10 if you're an employee and need to apply for Additional Statutory Paternity Pay (ASPP) and Leave if the mother or adopter of the child has died.

There is no longer a requirement for this form as Additional Statutory Paternity Pay and Leave was replaced by Shared Parental Leave and Pay for babies due or children placed for adoption on or after 5 April 2015.
